5 Items You Should Have at Your DOT Physical

1. Valid Photo Identification

To verify that you’re the same person who is applying for or maintaining a CDL, bring a photo ID to the appointment. Typically, a standard driver’s license will suffice to meet this requirement.

2. Vision & Hearing Aids

If you use eyeglasses, contact lenses, or hearing aids, wear them to the exam. During the DOT physical, your provider will need to ensure you can see and hear safely on the road.

3. List of Medications

DOT physicalBring a list of all medications you’re currently taking, their dosages, and the contact information for the prescribing doctors. Certain medications—such as those that are used to treat seizures or are controlled substances—may disqualify you or pose limitations to your approval. If you take any medications that may interfere with licensing, bring a letter of clearance from a doctor.

4. Medical Release Letters

Some medical conditions or disabilities may require further approval in order for you to pass a DOT physical. For example, individuals who take blood thinners, had a stroke, are prescribed sedatives, lost a limb, or have a pre-existing heart condition will likely need their doctor to write a letter stating that they are safe to drive.

5. Waiver or Exemption Certificates

Drivers planning on operating commercial vehicles between states may also be required to provide an FCMSA Waiver or Exemption Certificate if they have diabetes, seizure disorders, as well as vision and hearing impairments. To obtain one, submit a complete application to the FCSMA before your physical.
