To reflect the recent passage of the Paycheck Protection Program Flexibility Act (PPPFA), the SBA in conjunction with the Treasury Department has revised Form 3508  PPP Loan Forgiveness Application, as of June 16, 2020. In addition, the SBA has issued Form 3508EZ, a simplified forgiveness application form available to certain borrowers.
