Due to these unprecedented times Elberta Dental can now provide for you E-APPOINTMENTS for consultations or emergencies for both new and established patients. 

A patient of record can use Facetime or Skype to conduct these appointments. A new patient may download forms from our website, Elberta Dental, scan these documents and email them to us at elbertafamilydental@yahoo.com. Or, Dr. Leverett will also be glad to leave the needed forms taped to the office front door with your name on it. Once they are complete they may be emailed back or dropped off at the front door. 

In order for any prescriptions to be written a patient must be a patient of record. This is accomplished by submitting the new patient paperwork and having a consultation with the doctor. 

You may text Dr. Leverett at 251-604-6330 or leave a message on his voicemail at the same number. He will respond as soon as possible and appoint as soon as possible.
