If a tooth fails to heal following root canal treatment, it may still be possible to save it. Through a process called endodontic retreatment, dentists can remove the compromised filling and address the underlying issue that prohibited healing before sealing the tooth back up. This procedure is also used to treat teeth that healed properly but were later jeopardized by infection or decay. 

How Endodontic Retreatment Is Performed 

Retreatment starts by opening up the tooth to gain access to the root canal. This typically includes removing restorative components like crowns and core material. After clearing out the filling that was placed during the first procedure, your endodontist will clean the canals.

Using special lighting and magnification devices, your provider will then identify any issues that compromised the tooth, like unusual anatomy, extra canals, and infections. Once they’re taken care of, the canal will be sealed again. 

If the canals are so narrow that they’re impassable, your endodontist may need to make a small incision to reach down and seal the root properly. This will prevent infection and other complications. Placing a temporary filling in the tooth finishes the procedure. Shortly thereafter, you can top off the tooth with a new crown. 

What to Expect During Endodontic Retreatment

Excluding the placement of the crown, it can be completed during a single appointment. As for recovering, your dentist will provide you with specific instructions.

For example, you might be asked to take pain medication and to avoid chewing hard foods or ice while the tooth heals. And, if you required incisions, you may have to return to your endodontist about a week later to have the sutures removed. All swelling, pain, and discomfort should dissipate entirely within about two weeks.
