1. Photo Identification 

To prevent insurance and billing fraud, emergency room admissions staff may need to see photo identification. A driver’s license or state-issued ID is sufficient, although some may also accept other forms of identification.

2. Insurance Card

When you arrive at the ER, admissions will ask you about your insurance. Bring your insurance card and any other documentation relevant to your insurance status. Some hospitals allow you to produce a photocopy of your card; others will require the original. 

3. Medical History

In addition, have the names and numbers of your doctors and specialists.  

4. Legal Documents

Bring copies of your living will, health care proxy, and do not resuscitate (DNR) order. If you’re ever in critical condition, these legal instruments will ensure you’re treated according to your wishes. 

5. Emergency Contact Information 

When an emergency occurs, the ER staff will contact someone close to you. Everyone should have a list of emergency contacts that includes names, phone numbers, and relationships.
