What Are the Requirements?

There’s a limitation as to who can apply for a duplicate car title—only the registered owner, authorized agent, or lien holder. Here are the required documents you’ll need to submit.

  • Duly Accomplished Form: Also known as DPSMV 1799, this form contains details regarding the vehicle and request. The Duplicate Title Affidavit portion should be signed and notarized. As an alternative to notarization, you can sign it when witnessed by an employee from the Louisiana Office of Motor Vehicles or Public Tag Agency after presenting proper identification.

  • car titlesCopy of Registration Certificate: Along with the request, you must attach a copy of the registration certificate, which you can get from the Office of Motor Vehicles’ microfilm records. If there’s none available, note it on the application to avoid rejection.

  • Documentation Granting Authority: If the registered owner or lienholder is unable to request the car title in person, an appointed representative can do so on their behalf. They must present proper documentation, such as notarized power of attorney, which gives them the authority to transact.

  • Affidavit of Non-Possession of Title: For car titles with existing liens, this part of the application form should be completed by the lienholder and notarized as well. If there was a recorded lien, proof of satisfaction is likewise required.  

  • Payment: You must pay $68.50 for the title in addition to various handling fees mentioned on the Office of Motor Vehicles website.
