At Keen Dental Care we know that prevention is a key component of good oral health.  Most major dental problems can be avoided with proper preventative care and early intervention.  Unfortunately, many people who do not have dental insurance neglect going to visit the dentist for preventative care.  That is why we are excited to introduce our new membership program, the Keen Dental Club.  The modest monthly fee to join the Club will cover all your annual preventative dental care such as exams, cleanings and x-rays.  In addition, a Keen Dental Club membership also provides savings of 15% on all other dental services such as fillings, root canals, crowns, veneers, dentures and more.

Healthy Adult:   $30/month (savings of $77/year)

• 2 exams including annual Oral Cancer Screening
• 2 healthy mouth cleanings

Kids:  $25/month (savings of $101/year)

• 2 exams & 2 healthy mouth cleanings
• All necessary X-ray images
• 1 emergency exam
• Annual topical Fluoride varnish
• 15% off all other dental services provided by Keen Dental Care

Perio Adult:   $50/month (savings of $118/year)

• 2 exams including annual Oral Cancer Screening
• 4 periodontal maintenance cleanings
• All necessary X-ray images
• 1 emergency exam
• 15% off all other dental services provided by Keen Dental Care

Family 4-pack Plan:  $100/month

• 2 exams & 2 healthy mouth cleanings/year per family member
• All necessary X-ray images
• 1 emergency exam/year per family member
• Annual Oral cancer screenings for adult members
• Annual topical Fluoride varnish for child members
• 15% off all other dental services provided by Keen Dental Care
• Perio Adult Members are not eligible for inclusion in Family 4-pack Plan
• Services cannot be transferred between family members and each family member must be named at enrollment.
• Family 4-pack plan is good for up to four family members.

Membership Guidelines:

• Membership is for 1 year from date of enrollment.  Membership will automatically renew unless cancelled.  You will be notified of pending renewal at least 30 days before enrollment date.
• Fees for additional services are due at the time of service.  Services not paid in full will be billed at the non-discounted rate.
• Membership fees are non-transferable.  Unused services do not roll over and will be forfeited.
• Membership discounts may not be combined with other discounts or promotions.
• Members account must be in and remain in good standing (no past due balances or excessive missed appointments) to be eligible for discounts.
• Membership terms may only be altered at re-enrollment.  Membership terms will not be altered without prior notification.
• Must be under 13 years of age at time of enrollment to qualify for Kids Membership.
• Family plan discounts are only available for immediate family members (parents/legal guardians and their children).  Children over the age of 21 are not eligible for Family 4-pack Plan.
• Keen Dental Club is NOT dental insurance and cannot be used with dental insurance.
