Personal Documents to Fill Out Intake Paperwork

If you do not have insurance, bring along documents that show proof of your household income. The information can be used to determine your financial responsibilities and assistance options.

List of Medications & Medical History Reports   

After intake, a licensed clinician will conduct a Comprehensive Clinical Assessment to determine what type of treatment and counseling you need. They will reach this conclusion by asking a series of questions about your personal and professional relationships, emotions, and treatment history. The more information you can provide the better, so bring along a list of medications you are taking, as well as those you’ve taken in the past.
