In California, for instance, petitioners must submit a completed Petition for Appointment of Guardian of the Person, along with a separate sheet containing information about the child. This paperwork identifies the parties involved, explains why the court should appoint a guardian, and why you would be the best candidate. Along with the petition itself, you must also submit paperwork declaring your identity, stating whether the current guardians have given consent, and showing that you fully understand the responsibilities of guardianship.

Additional Copies

When completing your guardianship paperwork, complete at least three copies of each form, and ensure each is filled out correctly. One copy of each should be filed with the court, one is for your own records, and the third is to be served to the child’s current legal custodian. In some cases, grandparents and other living relatives will have to receive copies of the petition.
