Why do you need an Annual Wellness Visit?

The Annual Wellness visit is a time for you and your provider to discuss all of your healthcare needs and challenges.  It is the best way we have to prevent or delay medical issues before they become a problem.  Getting preventative care at an Annual Wellness Visit saves you time, money, and worry that comes with medical problems and can prevent unnecessary visits to the Emergency Department.  Preventative care can help improve your quality of life and keep you independent for a long time.

Do I have to pay for the visit?                              

No.  Medicare pays the entire cost of the visit if:

• You have had Medicare Part B coverage for more than 12 months
• You have not had a Medicare Initial Preventative Physical Examination or an Annual Wellness Visit within the last 12 Months

Is this just the same as my Annual Physical Exam?

No. During your Annual Wellness Visit you will work with your primary care provider to put together a plan for your health and wellness across the next year.  You will use this plan during your routine checkups to make sure you are staying focused on the most important parts of your health and wellness.  At these visits your doctor can look at your plan and see where to focus the most attention, giving you more time to talk about how you are feeling.

What will happen during the visit?

During your Annual Wellness Visit, we will gather as much information as possible about your health and update your electronic health record.  We will talk through all of your medical conditions, any medications you may be taking, and discuss any gaps in information.  We will also talk through any medical problems you may need to watch out for in the future, and how you can prevent them.  Although we may give you some preventative care like a flu-shot you will not be given an actual physical exam.

What if I have a specific problem I want to talk about?

If you have a specific health concert you would like to talk about, please schedule a separate appointment to discuss it.  Any specific health condition we treat during the Annual Wellness Visit may result in an extra charge.

Can I have a copy of my personalized care plan?

Absolutely.  We will discuss your care plan with you and send you home from the visit with a copy.  The copy can easily be taken with you if you see any other providers.

What should I bring to my visit?

When you come in for your Medicare Annual Wellness Visit, please bring the following.

• Any medical records you have including immunization records.
• Any family health history you have, in as much detail as possible.
• A complete list of medications you take, including vitamins, supplements, and herbs.  For each medication, write down how often and how much you take.
• A list of healthcare providers and suppliers currently involved in your care.
