This is a customer service driven management position; full-time, salaried based on a 40 hour workweek with core hours of 8:00 AM to 4:30 PM, Monday - Friday. Oversees Social Services department as well as performs administration duties as assigned. As a salaried position may extend beyond the core hours. Participates in policy development and assists in determining the most effective use of existing personnel and fiscal resources. Disseminates policies and procedures necessary for each department to function in compliance with all regulations, rules, and standards. Employee is responsible to plan, organize, develop and direct the overall quality of life program. Ensures Social Services and various other departments main focus is on honoring the values of person-centered living and resident directed practices. Collects and organizes data and performance tracking tools related to quality of life measures. Ensures staff engagement & participation in quality improvement measures. Hires, orientates, supervises, trains, motivates, evaluates, disciplines and schedules assigned staff. Provides implementation of Person Centered Care competencies through role modeling, coaching, supporting progress, recognizing strengths and needs. Assist with strategic planning, tours, and assists with marketing of campus. Organizes & participates in facility-wide assessment. Participates in staff meetings with other professional personnel. Assists and develops in-service topics coordinating with Staff Development Coordinator. Provides consultation and training when directed or necessary. Performs administrative duties such as completing necessary forms, reports, audits, studies, etc., in a timely fashion. Keeps abreast of current federal and state regulations, as well as professional standards, and makes recommendations on changes in policies and procedures to the Administrator. Keeps staff informed of regulatory requirements for skilled facilities. Together with Admission Coordinator, evaluates referrals and determines appropriateness of placement. Has knowledge of insurance, Medicare, Medicaid, Social Security and other financial support programs. Provides direction and/or assistance to resident or families with application processes as necessary. Ensure Social Worker is up to date and timely with documentation in resident medical record pertinent personal and psychosocial information, including social history, assessments, care plans, and progress notes. Maintains residents' rights and confidentiality of patient information; functions as Facility Grievance Official and is involved with Resident Protection. Coordinates investigation of all reported incidents of caregiver misconduct. Prepares or directs preparation of committee's reports to appropriate state agencies. Complete records of all reported incidents. Maintains record of "Lost Item Reports". *Graduation from an accredited college or university with a Bachelor's Degree in Social Work, Health Care Administration, or related field and two years' supervising experience in health care services with geriatric patients or in a long-term care facility. *Must be knowledgeable of State and Federal rules and regulations regarding the operation of a nursing home facility. *May consider other relevant combinations of training and experience. *Preference for a current Social Worker or Wisconsin Nursing Home Administrator's license. *Preference working with case management services and Medicare, Medical Assistance and Social Security Systems. *Requires excellent interpersonal, oral and written communications skills. *Must be available for emergency and non-emergency calls on weekends/evenings. *Successful completion of criminal record check in compliance with the Wisconsin Caregiver Law.
