Here at Home Helpers in Westminster, Colorado, we have a well -defined process in place to ensure all care partners and prospective clients understand our home care services and the appropriate procedures for us to deliver our services.
We typically receive a phone call, usually by the adult child, after a crisis has happened in the home. Maybe Mom fell and is now in the hospital. Maybe Dad is lonely or is losing his cognitive faculties. At the first phone call, I always ask, “Tell me a little bit about what is going on?” And then comes the flood of information by the prospective client. It’s typically a great call and this first call is one of my favorite parts about this job because right away, I’m offering the family member peace of mind that everything is going to be OKAY and we will do everything we can to make sure Mom or Dad can stay in their home safely.
Once the start of a relationship has been established over the phone, a home care assessment is scheduled to determine if our agency can meet the client’s needs in a safe and efficient manner. This is my SECOND favorite part of my job because I LOVE meeting and helping new families who don’t know where to turn. I pride myself of my knowledge of the senior care industry and the resources I may be able to share with families. If the services required are outside the scope of what we can provide, I do not hesitate to share resources with the family. Often times, we are the appropriate provider, but work well with additional services such as hospice or home heath.
We then develop our individualized service plan. We discuss exactly what services are needed, what days and times work best for the client and what type of caregiver would have the best relationship with Mom or Dad. Or Grandpa or Grandma. Or a Family Friend needing that little help in the home. I have exceptional caregivers that I am extremely proud of and they all adapt well to different and sometimes, difficult or stressful situations.
I draw up a service agreement. This IS NOT a contract. It’s a simple form that speaks to a few of my policies such as escorting clients and schedule changes. It’s not scary at all and takes little to no time to complete.
Then comes another fun part. I find the BEST caregiver that I work with who will best fit the needs of the client and we discuss all the services that need to be implemented. Working with my caregivers is truly a great part and always serves to remind me of why I got into this crazy business in the first place. There are truly people in the world that CARE ABOUT OTHERS. The caregiver follows the care plan and observes, reports and records any changes in the client’s condition so that we can communicate to the family if any additional resources need to be contacted. This service delivery process is so important because the process lets us do what we do best and that is to help others to age in place and keep them in their home!
