  • What To Bring: Bring your driver's license or another form of photo identification along with your insurance card. They will also need the birth date of the primary subscriber on your insurance. If you have X-rays or MRI/CT studies, it is a good idea to bring these to the appointment as well. 
  • orthopedic doctorsWorkers’ Compensation: If you were injured on the job and are filing a workers’ compensation claim, you will need to provide information related to your insurance carrier, claim number, date of injury, and adjuster's name and contact information.
  • What To Wear: It is advisable to wear comfortable, loose-fit clothing so the orthopedic doctors can thoroughly examine you. If you have shoulder problems, please wear a sleeveless shirt or a tank top. If you have knee problems, please wear shorts. For OMT problems, wear shorts or loose-fit pants.
  • What To Know: IMUA Orthopedics, Sports & Health understands that life is busy and you may need to cancel an appointment. As part of their cancellation policy, patients must notify them 24 hours in advance. If you do not give prior notice or are a no-show, they may charge a $30 fee. Multiple no-shows may require a temporary suspension of services.
